This form must be filed if employment expenses are being claimed in Form T777, which updates to line 22900 of the T1 jacket.
The individual can enter the conditions of employment, but the employer should answer these questions. He has to complete and sign Part B of the form. If the individual has more than one employer, have each employer complete and sign a form. You do not have to submit this form with the return. However, the individual must keep it in case the CRA asks to see it.
