Application for the Guaranteed Income Supplement or Statement of Income for the Allowance or Allowance for the Survivor

This form is used to calculate income qualifying for the Guaranteed Income Supplement and determine if the taxpayer is eligible for this supplement for the period of July 2022 to June 2023. This form must not be sent to Service Canada.

Only the official version of Form SC-ISP-3025 downloaded from the Service Canada Web site can be used to apply for the Guaranteed Income Supplement. Therefore, the data calculated in this form will have to be re-entered in the official version of the form.

Guaranteed Income Supplement (GIS) and Allowance payments are normally based on your previous year's income. Generally, you report your income and use your deductions in the same way you do on your Canada Revenue Agency (CRA) federal Income Tax and Benefit Return. Your entitlement for these benefits will be reviewed annually. A renewal application will be sent to you if necessary. Please file your Federal income tax return on time each year as we use this information to determine eligibility. Also, contact us if there is a change to your marital status and/or income.
